Position Description


REPORTS TO: Associate Director

OVERALL SUMMARY:  The Care Manager is responsible for assessing the needs of older adults in West Marin, and for providing appropriate referrals and support to help the elderly and frail remain safely and with dignity in their homes. Clients and their families may be referred to our assisted living facility, Stockstill House, when appropriate.


  • MSW, LCSW, or RN desirable, 4 year college degree, background in social work, nursing, or homecare required.
  • Good interpersonal skills.  Able to handle sensitive issues with good judgment, tact, and discretion and maintain strict confidentiality.  Able to work with a wide variety of people including clients, other agency personnel and service providers, medical practitioners, home care works, and staff.
  • Good oral and written communication skills, including strong telephone skills and working with the hearing impaired.   Pleasant manner when dealing with others.
  • Good computer skills, including word processing, e-mails, and using spreadsheets for reports and data management.
  • Background in social work, homecare, or nursing required.
  • Possess a valid CA driver’s license and state required car insurance as required by WMSS insurance policy.  Able to use personal car to visit clients.


The Care Manager must demonstrate the ability to:

  • Conduct telephone interviews to determine needs and appropriateness of our services. Make referrals to other organizations, if outside of our geographic area.
  • Conduct accurate client assessments, including social, environmental, nutritional, and physical conditions of the client and his/her living situation.  Review fee for service with potential clients and their families.
  • Create effective care plans for clients and their family, addressing client and family needs, set clear boundaries and refer to other resources when appropriate. Revise the care plan, as needed.  Engage family members in the care of the client when appropriate.
  • Mediate between clients, family members, and caregivers, as needed.  Offer emotional support and refer to professionals when appropriate.  Report problems or issues to supervisor in a timely fashion.
  • Identify self-neglect or abuse and take appropriate action.  Report to authorities, following the guidelines of a Mandated Reporter.  Review with immediate supervisor and complete required paperwork for Adult Protective Services, including copies to Executive Director.
  • Refer vetted caregivers to clients and families from Registries or Home Care Agencies. Help families understand employment ramifications of hiring a caregiver.  Recommend grants, if appropriate, and complete required paperwork.
  • Complete paperwork and reports accurately and in a timely manner, including monthly statistics, assessments, quarterly HDM assessments, care plans, case notes, grant requests, fee for service contracts, and other required data.
  • Participate in community outreach, educational, and social activities, including workshops, congregate luncheons, and fundraising events.
  • Attend monthly meetings with the Older Adult Network, Staff and Case Management meetings, and in-service workshops.
  • Undertake other duties as assigned.


This is a non-exempt position and salary is dependent on knowledge, skills, ability, and experience.

Stockstill House is an eight (8) bed, Assisted Living Residential home in Pt. Reyes Station. We are looking for a CNA who wants a break from the more institutional setting of elder care, has experience with memory care and who is willing to make a long-term commitment.

  • Enjoy a restful, reverse commute through exquisite countryside with no “rush hour”.
  • Located 30 minutes from Petaluma & Novato and 45 minutes from San Rafael.
  • Stockstill House is surrounded by a lovely, large yard with a walking path, brick patio, raised garden beds & amazing views of the West Marin landscape.
  • We work in teams of 2 to 3 caregivers for daytime shifts and 1 caregiver for the overnight shift.

West Marin Senior Services (WMSS) is a nonprofit organization that provides support services to seniors living independently. WMSS owns and operates Stockstill House, an Assisted Living Residence, for up to eight (8) seniors who need continuous care. Part-time & full-time positions available. Benefitted position at 20 hours per week.
