What are Care Managers?
Care managers work with seniors and their families to develop care plans for those at risk, to assure they can remain safely in their homes. We provide case management services to over 300 seniors living in West Marin each year. Care Managers support through home assessments (evaluating the safety of one’s home), care plans, and ongoing care monitoring. We connect those in need to food programs, financial assistance, health insurance options, housing programs, home health agencies and many other services
